In a recent study BMC Infectious Diseases, researchers investigated the eradication rates of Helicobacter pylori using levofloxacin- and clarithromycin-based regimens in patients previously treated for coronavirus disease 2019 . Background Helicobacter pylori affects half of the world’s population. Chronic gastritis is considered the primary organism responsible for several stomach cancers and peptic ulcer diseases. It has also been associated with extraintestinal disorders such as treatment-resistant iron deficiency anemia, vitamin B12 deficiency, and immune thrombocytopenic purpura. H. pylori eradication rates may be affected by the use of antibiotics during the COVID-19 pandemic. about the study In this study, researchers evaluated efficacy of clarithromycin for the treatment of H. pylori compared to levofloxacin-based treatments in antibiotic-naive individuals after COVID-19 pandemic. The study included a randomized controlled trial conducted from March 21, 2021 to September 30, 2021, with patients recruited from the outpatient centers of Alexandria University hospitals and patients referred by doctors working in outpatient and inpatient facilities. Eligible participants included patients aged 18 to 65 years newly diagnosed with H. pylori infection and previously treated for COVID-19. Diagnosis is based on detection of positive H. pylori stool antigen, rapid urease test, urea breath test, or H. pylori during histological analysis of gastric biopsies. The test was performed in subjects with peptic ulcer, prior history peptic ulcer, history or presence gastric cancer, dyspepsia, chronic use aspirin or pain relievers, and endoscopy for evaluation upper gastrointestinal symptoms. The primary complaint of each patient was recorded. The first group was given amoxicillin, clarithromycin and esomeprazole, the second group was given esomeprazole, levofloxacin and amoxicillin. Download a free copy All patients were provided with comprehensive clinical and laboratory examinations, as well as comprehensive historical data, including demographic information and social history of alcohol or smoking. Patient compliance was estimated by numbering the remaining tablets at predetermined intervals. All patients were asked to answer a questionnaire regarding adverse drug reactions. The severity of each symptom was graded from absence to severe . After six to eight weeks treatment, at least four weeks after discontinuation of antimicrobials, and without PPI for at least two weeks, the team evaluated the eradication H. pylori. The study involved 270 participants, 135 in each arm. Approximately 19 patients from the clarithromycin cohort and 18 patients from the levofloxacin cohort discontinued treatment within two to four days due to adverse outcomes or lost follow-up before evaluation of H. pylori eradication. Then, 116 participants from the clarithromycin cohort and 117 participants from the levofloxacin group were evaluated. The mean age of the participants was 41.9±13.0 and 58.8% were male, 63.4% were married, 88.0% resided in metropolitan cities and 60.1% had no history of chronic disease. The rest of the patients showed compliance over 80%. The team noted that there were no statistically significant differences between the levofloxacin and clarithromycin-based regimens in terms of baseline characteristics, primary complaint, and diagnostic test used. Approximately 25.5% of the patients examined were smokers, whereas none of them reported drinking alcohol. The overall H. pylori eradication response rate was 69.53%. Intent-to-treat and per-protocol analyzes showed that 64.44% and 74.36% of patients treated with the levofloxacin-based regimen exhibited a treatment response. By comparison, those treated with the clarithromycin-based regimen had a treatment response of 55.6% and 64.66%, respectively. However, these differences were not statistically significant. Additionally, the team noted that there were no statistically significant changes between adverse events reported by the two treatment groups. Solution Study findings showed that both levofloxacin and clarithromycin-based regimens resulted in less than acceptable eradication rates among participants with a history of prior COVID-19 treatment. The researchers believe this finding should be a cause for concern regarding the rise of antibiotic resistance among these individuals and the general public. This indicated that increased resistance could negatively impact H. pylori treatment costs and increase the risk of H. pylori-related disease. Additional research is required to elucidate the precise mechanism of antibiotic resistance to H. pylori in such patients.
